This policy describes information provided through carewaycds.com and CareWay CDS LLC office text messaging. It does not replace any privacy notices provided separately for care services.

Information you provide

Our Request Care form collects your name, phone number if provided, email address, service requested, comments, submitter role, and consent choices. We use these details to respond to inquiries and coordinate requested office communications. Please do not submit medical records, diagnoses, or other sensitive personal information through this form or by text.

Office text messaging

If you choose to receive texts, CareWay CDS may send nonpromotional messages about care inquiries, appointments and reminders, caregiver scheduling, and related office coordination. SMS consent is optional and is not a condition of requesting or receiving services. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for assistance. Mobile information and consent

No mobile information will be sold or shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with third parties or affiliates for their own use. Service providers may process information only as needed to support our communications, including message delivery, and not for their own marketing.

Website and service providers

Our website and inquiry form use WordPress, Hostinger hosting, and Contact Form 7. Inquiry messages are directed to our office email. Hosting, email, and messaging providers process information needed to operate these services. Routine technical information may be processed in providing website services. Contact our office with questions about how your inquiry information is handled.

Your choices and questions

You can leave SMS consent unchecked and still submit an inquiry. Contact our office to update your contact information or ask about information you have provided. Text messaging is not monitored for emergencies. Call 911 in an emergency.
